I. Introduction

This waiver is to inform participants of the nature and risks involved in the dance classes provided by Shayelle Lajoie. Participation in any dance activity is voluntary and requires that the participant abide by all applicable rules and standards of conduct.

 

II. Participant Acknowledgment

I understand that participation in adult ballet involves certain risks, including, but not limited to, physical injury, emotional stress, or other personal harm. I agree to participate in these activities fully aware of these risks.

 

III. Assumption of Risks

I voluntarily assume the known and unknown risks associated with adult ballet, releasing Shayelle Lajoie and Dance Energy Studios from all liabilities and claims arising from my participation.

 

IV. Compliance with Rules

Adherence to Rules: I agree to comply with all rules and instructions provided by the instructors.

 

V. Emergency Medical Treatment

In the event of an injury, I consent to the procurement of medical treatment deemed necessary by my instructor staff or its appointed representatives. I am fully responsible for any associated treatment costs.

 

VI. Image and Likeness Release

Unless I email Shayelle Lajoie to state otherwise, I grant Shayelle Lajoie the right to photograph or video record myself during dance classes. These images and videos may be used for promotional, educational, or informational purposes without compensation.

 

VII. Legal Statements

• Confirmation of Understanding: By participating in Adult Ballet with Shayelle Lajoie, I confirm that I have thoroughly read and understood its content.
• Agreement of Free Will: I agree to the terms of this waiver on my own free will, without any external pressure or coercion.
• Legal Binding: I acknowledge that this document is legally binding, and I understand the legal implications of agreeing to it in full by participating in Adult Ballet with Shayelle Lajoie.
• Release of Liability: I acknowledge that by participating in Adult Ballet with Shayelle Lajoie, I release Shayelle Lajoie from any liability associated with my participation in the dance classes.
• Voluntary Agreement: I affirm that my agreement to this waiver, by participating in Adult Ballet with Shayelle Lajoie, is voluntary and without any duress or undue influence.

 

VIII. Acknowledgment of Understanding

I have read this waiver of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that by participating in Adult Ballet with Shayelle Lajoie, I am agreeing with this waiver freely and voluntarily and intend this to be a complete and unconditional release of all liability to the greatest extent allowed by law.
